• Single Status Letter for citizen of Indonesia from Kelurahan;
• Certificate of Decree Absolute if divorced;
• Death Certificate of the Former Spouse if widowed;
• Copy of passports of two witnesses;
• 4 (four) pieces of photograph sized 4x6 posing the groom in the right side of the bride on red or blue background (available in Bali);
